Medicare and Private Health for Chronic Disease Management Do you have a chronic medical condition? You may be eligible for 5 subsidized sessions by an Accredited Exercise Physiologist who can plan and prescribe an exercise program suitable for your medical condition. See your GP and ask for a Chronic Disease Management (CDM) or Enhanced Primary Care (EPC) Referral and come in to see us. We DO NOT charge a gap fee! For private health, You may be eligible for a rebate depending on your level of cover. Please contact your provider for further details, or send us an enquiry.
